Network Program - Support Group Meeting Summary Form

This form is to be completed after each support group meeting by all support group facilitators who attend Willow Facilitator Training Workshops.

Thank you for your assistance in helping us to maintain current data on the dynamics and development of breast cancer support groups.  At a later date the Support Group Meeting Summary form will be available in a format which can be completed and submitted from this site. Until then you can use the MS Word form in two ways:

a) Save the form, complete it on your computer and submit via email, as an attachment, to dvandezande@willow.org
b) Print the form and complete in writing, and mail to:
